This week’s episode of NXT featured a big title change as Thea Hail managed to defeat Blake Monroe to become the new NXT Women’s North American Champion. Hail picked up the victory after she did a spring board senton off the ropes and landed on Blake Monroe.
Hail then pinned Monroe and the referee counted to three, but there was an awkward pause before the bell rang. After the bell officially rang Blake Monroe’s music briefly played despite the fact that Thea Hail won the match. The commentators indicated that there was some confusion regarding the result of the match, but Thea Hail was awarded the NXT Women’s North American Championship.

Dave Meltzer of the Wrestling Observer is reporting that Thea Hail beating Blake Monroe for the NXT Women’s North American Championship was an unplanned title change.
That was in fact an unplanned title change. I can't think of the last time that happened.

It’s also worth noting that TC of WrestleVotes reported during the show that discussions were underway regarding how to handle the unforeseen title change. The expectation was that changes would be made to the remainder of the episode.
I'm told by a source behind the scenes at NXT right now that discussions are underway as to how to handle the unforseen Women's North American title change in the match between Monroe Blake & Thea Hail.
Changes to the remainder of the show are expected.

Later in the show Thea Hail was interviewed backstage and Jordynne Grace congratulated her. Blake Monroe confronted Thea and the former champion was emotional as she stated that she kicked out. Grace told Monroe to let Thea Hail have her moment, and Monroe demanded her rematch before she walked off.
